embryonic stem cells come from what kind of embryo?

I know that embryonic stem cells are stem cells derived from the inner cell mass of an early stage embryo known as a blastocyst. as Wikipeida says it. But in what kind of embryo do they obtain the stem cells from? Are they the embryos fertilized in lab or donated by people?
